A Study on Epidemiological characteristics and Comparative Validity among the Case Finding Methods of Nosocomial Infection in an University Hospital, Seoul


Abstract
Nosocomial infection is an impending problem in public health. This country, however, has no study result about effective surveillance system in terms of sensitivity and specificity of the case-finding methods currently in practice. This study
was
carried out to investigate epidemiological characteristics and to compare the validity among the case finding methods of nosocomial infection in an university hospital, Seoul. Study subjects were patients who were febrile, took antibiotics, and
had
positive microbiological tests. The data were collected from August 27 to December 19, 1993.
@ES The results obtained were as follows;
@EN 1) The total number of nosocomial infection identified by CDC guideline during one week was 71 cases among 2644 subjects. Of these the patients who had high fever( more than 38¡É)were 42 cases(59.2%), took antibiotics were 58 cases(81.7%),
and
whose
microbiologic tests reported to be positive were 42 cases(59.2%).
2) Reletive validity showed various value by method.
According to the results, the most accurate method appears to be the case-finding methods based on febrile patients or microbiologic tests reported to be positive; Sensitivity was 84.5%, specificity 83.9%, positive predictability 12.7%, and
negative
predictability was 99.5%.
